要帮公司CUT掉电话LINE的信怎么样写？

我是ADMIN 部门的。
我的MANAGER 跟我说之前的EX-EXECUTIVE 开了一条宿舍的电话LINE， 但是现在没人住了，所以要CUT 掉。
问题是他说通常是去申请那条LINE 的人才可以CUT掉那个LINE，但是那个人都没再我公司做了，所以要我ON BEHALF 写封信给TELEKOM。
请问各位，那封信要怎么样写？要写些什么？程序是如何的？拜托各位帮帮忙 |
